As seen in a clip circulating on X (formerly Twitter), Charles and Camilla took part in a special ceremony in the island state on Thursday, Oct. 24, when many fans noticed the King looking "tired." Some even thought he may have been nodding off, as his head appeared to bob at one point before Camilla got his attention.

Many fans on social media instantly became concerned over the status of Charles' health, especially after he was diagnosed with cancer at the start of this year.

"The man was actually falling asleep," one person on X suggested. "You see his head going down slightly and then catches himself, and his head gives a little jerk and awake he is."

That same user said the engagement should have been given to Charles' son, Prince William, 42.

"Charles is napping," someone else added, insisting the monarch "really needs to go home."

"What is going on with his hands? They are bright red and swollen," a different user pointed out.

However, others made a point to defend Charles amid his ongoing health battle, as one person gave him credit by saying, "he's out there doing his job while undergoing cancer treatment."

Another photo taken during the King's arrival in Samoa also sparked concern from fans, as one person said he looked "really tired and old" in the snap.

As Parade previously reported, Charles received permission from his doctors to "pause" his cancer treatments while he traveled to Australia and Samoa, a trip that began on Oct. 18.

He is expected to restart his treatments when he returns to the U.K.
