For dermatologist Sandra Lee, MD, a.k.a. Dr. Pimple Popper, every extraction she does is like a treasure hunt. And in her latest Instagram video, she is extracting a huge blackhead that she's sliced open to look like an 'X marks the spot' on her patient's skin. Sure enough, she's about to extract the massive blackhead for all that buried treasure.

"??marks the spot... for BIG blackheads! #drpimplepopper #dpow," Dr. Pimple Popper captioned the video.

The video starts with an image of the blackhead pre-pop, and it has an 'X' on the top of it ready to be extracted. It may not be particularly creative, but it is convenient.

"I'm going to squeeze it, you're going to feel some pressure..." Dr. Pimple Popper tells her patient as she gets to extracting with one of her comedone extractor tools."These are more dried up so you kind of have to squeeze them to get all of that out. It's behaving though."

As she presses down, the gunk that comes out is brown and black, and Dr. Pimple Popper explains to her patient why it's coming out that color.

"The reason they're so dark is because they're oxidized...they've hit oxygen," Dr. Pimple Popper explains. "The ones that are trapped under there and don't get air exposed to them, that's what we call whiteheads."

She goes in again with the comedone extractor, removing more dark gunk from the massive blackhead that has a hard texture.

"Yours was pretty hard...it's moderately hard. I've felt harder ones," says Dr. Pimple Popper. Who knew there was a competition going on?

Once she's done squeezing, she switches over to a pair of tweezers to remove a sac that's still in the patient's skin.

And her Popaholics loved every bit of it.

"So much inside ??????," one commented, and another wrote," That’s not big...it’s HUGE ??."
