In a new Instagram video, Dr. Pimple Popper — aka, dermatologist and TLC host Dr. Sandra Lee, MD — pops a series of three whiteheads behind a patient’s ear, including a particularly large specimen. Though the clip is quick, you’ll leave this one disturbingly satisfied.

“Look what we have ‘ear’,” one fan cheekily commented on the video. What we have are whiteheads, which form in a hair follicle just under the top layer of skin. Whiteheads are filled with dead skin cells and sebum, or oil secretion. Though dermatologists say whiteheads are hard to prevent, they are often easy to extract, as Dr Lee proves in this video.

In the clip, Dr. Lee extracts the whiteheads with a simple press of her comedone extractor. The small metal instrument helps the famous doctor reach the delicate and hard-to-reach area behind the ear, which the patient says he has been unable to reach on his own.

In the video, Dr. Lee pops one especially large whitehead, which snakes out like a small worm. “The big one was soooo ready,” one fan commented. And man, she’s right about that one.
